To determine eligibility, you will need to complete an application, provide a copy of your child’s ORIGINAL birth certificate, copy of your child’s CURRENT immunization records and income information for your family (1040 tax form, pay stubs for the last year or a TANF award letter).  Your child must have a medical exam completed within the first 12 months and a dental exam completed within the last 6 months.
